For example, if seepage is noted, it is important to note whether this is new seepage, a change in seepage, or seepage that has existed for a long time and doesn't pose an immediate threat to the integrity of the dam. How do I know if something is wrong with my dam? Formal inspections are comprehensive searches for evidence of deterioration of materials, developing weaknesses, and unsafe hydraulic or structural conditions. Samples of checklists can be found in the Owner's Guidance.

Types of Inspections. Risk Assessment. In addition to completing the dam inspection formthe engineer must also provide a cover letter which summarizes the inspection and includes their official Professional Engineer stamp and signature. The form includes areas for you to indicate the condition of the dam and its appurtenant structures. Waiver requests cannot be submitted for dams with a height of 50 feet or more.

It is important that this data also be compiled in a systematic manner and placed in the permanent file. If any repairs must be made to the dam or its appurtenant structures, you must receive the approval of the Safe Dams Program. To clarify, all of the quarterly reports for the year should be mailed in to the Safe Dams Program at one time by April 30 of each year. Field Sketch A good practice in addition to filling out the inspection checklist is to draw a field sketch of observed conditions. Photographs of important features must be included in your report. As part of the engineer-led inspection report, the engineer must complete the dam inspection form or equivalent and include a cover letter which summarizes the inspection and contains their official Professional Engineer stamp and signature.

Routine inspection and maintenance allows early detection of many problems that may occur with a dam. You may hire an Engineer of Record to investigate the extent of damage. A quantitative risk analysis yields Periodic dam safety inspection report book numerical estimate of the risk of adverse consequence, multiplying the probability of load times the probability of dam failure.

The large number of dams that are 30 or more years old is a matter of great concern. Major indicators of possible dam failure include heavy seepage, overflowing water overtoppingsink holes, sand boils, muddy water flow from drains, and unusual water level.

Any dam which is less than 50 feet tall and that the owner has submitted four consecutive quarterly inspections qualifies for the waiver; however, the waiver may be denied for reasons which include, but are not limited to, inspection reports showing deficiencies that have not yet been corrected or enforcement actions within the past 24 months.

Use of the checklist will provide consistency for inspections and a more efficient and accessible review process. Other symptoms of failure include, but are not limited to erosion, surface cracks, settlement, movement, and wetness on the slope of the embankment.

They use the dam height or the water storage capacity at maximum water storage elevation, whichever yields the larger size classification, to determine the size of a dam. In the last decade, several major dam failures have increased public awareness of the potential haz- ards caused by dams. The dam owner is required to submit quarterly reports to the Safe Dams Program; however owners should contact the Safe Dams Program immediately when a potential failure is noticed.

What is a waiver request? Why are inspections by an engineer required? Your waiver request may be denied for reasons which include, but are not limited to, inspection reports which show deficiencies that have not yet been corrected, Division enforcement actions within the past 24 months, submittal of the request past the October 1 deadline, or you may not qualify to submit a waiver.

As part of the engineer-led inspection report, the engineer must complete the dam inspection form or equivalent and include a cover letter which summarizes the inspection and contains their official Professional Engineer stamp Periodic dam safety inspection report book signature.

Photographs should be taken in color and date stamped. The quarterly inspections should then be submitted to the Safe Dams Program on a yearly basis by April 30 of each year. Also, many owners accompany DEC engineers during their periodic inspections.

Formal inspections are comprehensive searches for Periodic dam safety inspection report book of deterioration of materials, developing weaknesses, and unsafe hydraulic Periodic dam safety inspection report book structural conditions. The checklist was designed to take into consideration the wide variety of dams in use in Wisconsin and covers all aspects of a dam inspection for which the Department needs documentation.

With this knowledge, quick and economical remedial actions can be developed and implemented, or avoided altogether. It is important that this data also be compiled in a systematic manner and placed in the permanent file. We typically perform this assessment as part of a dam safety inspection report.

Dams must be inspected by an engineer at least every two years between October 1 and March When do I need to send my reports to the Safe Dams Program? Many of the older dams are characterized by increased hazard potential due to downstream develop- ment and increased risk due to structural deterioration or inadequate spill- way capacity.

What must be included in the report? According to the Georgia Safe Dams Act and Rules for Dam Safety, it is the responsibility of the dam owner to inspect and maintain their dam.
